The Application Process

A COMPLETED APPLICATION TO SSDSRV INCLUDES:
- A completed Application Form with (optional) photo
- A non-refundable application fee of $100
- A signed Transcript/Records Release Form
- A photocopy of your child’s birth certificate (and conversion certificate, if applicable)
- A scheduled visit for your child’s interview and informal screening arranged through the Admissions Office.
- Completed Tuition Assistance Forms (if applicable)
The deadline for all forms is January 31, 2008. Late applications will be considered on a space available basis.
